To request anatomical models for use outside of the library, open the request form in a new tab or access it below to provide your contact information and your requested model names with barcodes.
*Note: Opening the request form in a new tab allows for toggling back and forth between the model information and the request form.
Library staff will email you to confirm your request.
You may also visit the library (HSEB 3rd floor) and request to checkout anatomical models in person.
